Waiver

The digital waiver must be signed once per playing year, which runs from tryouts through the spring season.  Each child participating in an Anglers FC program must have a signed waiver; however, multiple children in the same family can be put on the same waiver.  Please click the button, enter your name and email address, and complete the form sent to your email inbox in order to review the Activity Release, Acknowledgement of Religious Liberty, and optional Media Permissions.

Activity Release

I consent to allow my child(ren) to participate with Anglers Football Club (the “Club”), including the following activities among others:

Activities: Activities that will occur within the entire scope of the sports of soccer and futsal including all events organized as a team after the activities.

I understand that these activities and the facilities where they are conducted involve inherent risks. Nevertheless, I want any registered child (and myself if registered) to have the opportunity to participate in them, and I am executing this Activity Release in exchange for that opportunity.

Waiver, Release, and Indemnification: I, individually, and in my capacity as parent, guardian, or next friend of any registered child, waive, release, indemnify, and promise not to sue Anglers Football Club, its employees, contractors, board members, and volunteers (collectively, “Released Parties”) from all demands, claims, or liability, in law or in equity, including the Released Parties’ own negligence, that have arisen or may arise from the related activities, and that involve any damage, loss, or injury to me, my spouse, any registered child, my property, my spouse’s property, or the property of any listed child. I fully assume the risks associated with participating in the designated activities. This waiver, release, indemnification, and promise not to sue does not apply to claims of criminal conduct, gross negligence, or intentional acts.

Medical: In case of medical need or injury, I understand that an authorized Anglers Football Club representative will make every reasonable effort to contact me (in the case of an injury to my child). In the event that I cannot be reached, I authorize any representative of the Club to arrange for medical services for me or for any registered child. I will be responsible for any medical and related expenses for me or such child. Any provider of care can rely on this Consent as authority to treat such child as appropriate and to bill me directly for the costs thereof. I understand that an authorized Club representative will hold any medication for my child until needed or scheduled, at which time it is my or such child’s responsibility to inform an authorized Club representative the medication needed. I shall be responsible for communicating any relevant medical conditions to the Club through email or phone.

To revoke this agreement, whether in part or in full, I must notify the Executive Director in writing in advance of the event.

Acknowledgement of Religious Liberty

I acknowledge that Anglers Football Club staff lead from a Christian worldview and that my child(ren) may be exposed to Christian values while participating in Activities that will occur within the entire scope of the sports of soccer and futsal including all events organized as a team after the activities.  This exposure may include, but is not limited to, Bible verses, Christian teachings, prayer, and encouraging words.

Anglers Football Club staff lead from a Christian worldview, but we are committed to bringing players and coaches from various religious beliefs, ethnicities, and backgrounds together through the sport of soccer.  We believe that all peoples, nations, and tribes are created in the image of God, that is, that all people are of sacred worth.  We are for love, grace, and compassion to all individuals.  There is no place for hatred or racism in our soccer community.  We are committed to fostering respect and diversity within our club.

Media Permissions

Anglers Football Club (the “Club”) requests permission for your child(ren)’s image, voice, video, work, and first name, to be published publicly on Club websites and social media pages or any other public website for news media purposes. Player images, voices, videos, work, and first name, may be used to promote or represent the Club, its programs, and its events, in the news media including, but not limited to, television, print, and on the web.

Accordingly, the Club will not post player images, voices, videos, work, or first name on Club websites, including Club social media pages, or any other public website for news media purposes without this signed consent from you as the parent or legal guardian. This permission will be applicable to use of student information and will remain in effect until removed from the website or until consent is withdrawn. As parent or legal guardian, you may withdraw your consent at any time by sending a written notice to the Executive Director.

By voluntarily signing these Media Permissions in the digital waiver, you grant the above permissions and agree to the following:

  • I agree to the use of my child(ren)’s image, video, voice, work, and first name to be used to promote or represent the Club in credible news media not affiliated with the Club and on Club websites and official Club social media pages (Instagram®, Facebook®, LinkedIn®, YouTube®).
  • In addition, I agree to release and hold harmless Anglers Football Club and its employees, contractors, board members, and volunteers from and against any and all claims, demands, actions, complaints, suits or other forms of liability that shall arise out of or by reason of, or be caused by the use of my child(ren)’s picture, voice, video, work, or first name on the Internet.

Signed Media Permissions are not required to participate in Club activities.
